I may (fingers crossed) be involved in organizing an open beta test of a game for a contest. Does anyone have scans of, or know where to find scans of, the surveys from past beta tests? If so, from those that are in Japanese, do you have the translations?

sounds like you're talking about a user test, or usability test - not a beta test. if you do a google search, you should find what you're looking for.
